Please be aware we are still open but , like all services, how we are working has changed.

All patients now receive an initial telephone appointment with the GP

GPs will conduct your consultation via telephone or video call.

Sometimes the GP will also ask you to send clinical pictures or photographs to the surgery. This helps with dealing with your query from the safety of your home.

We understand that face to face assessments are required. Patients triaged by the GP to a face to face assessment are asked to attend wearing a mask to our surgery at designated times.  For staff and patient protection, healthcare professionals will wear full personal protection equipment (PPE).

Online Consult allows you to submit questions about a health concern online.

We aim to respond to a submitted query within 24 hours , often much sooner.

You can receive feedback by text, email, telephone, video or face to face if follow up is needed.

Like To Register?

New to the area and would like to join our practice?  Fill in our registration forms available from: https://hazeldenemanchester.nhs.uk/new-patients/

  • Please call into surgery to collect forms  or apply via the website.
  • Two forms of ID are required  eg Passport/Driving Licence including the photo together with a current household bill showing the current address at which you wish to register.
  • Once you have completed the forms and produced your ID as above the reception staff will pass your details onto one of the GP’s at the practice.
  • Please allow 48 hours for a reply, please either phone or email us to see if you have been accepted by the practice.
  • If you have you will then be offered an optional new patient medical by our healthcare assistants.
