Appointments

Online Consultations 

You are able to complete the online consultation form out and have a response from the practice within 2 working days. this means you do not need to call the practice at 8am for  none urgent appointment. we will contact you with an appointment, just let us know what your requirements are on the form. https://www.online-consult.co.uk/org/hazeldene-medical-centre

 

Face to Face Appointments 

You are able to call the surgery to book a face to face appointment or request a face to face appointment via an online consultation form, appointment are available to prebook or on the day for urgent care. Face to Face appointments are available with the GP, Registrars, Physician Associates, Nurse, Nurse Associate, Mental Health Practitioner, Physiotherapist and Pharmacist.

 

Telephone Consultations

You are able to call the surgery to book a Telephone consultation or request a Telephone consultation via an online consultation form, appointment are available to prebook or on the day for urgent care. Telephone Consultations are available with the GP, Registrars, Physician Associates, Nurse, Nurse Associate, Mental Health Practitioner, Physiotherapist and Pharmacist.

 

Nurse / Nurse Associate / HCA appointments 

All appointments can be booked up to six weeks in advance (subject to availability). Some appointments are only bookable on the day and are designed for emergencies and the more urgent cases. Patients requesting same day appointments should contact the surgery from 8am, contact NHS 111, attend a walking center or attend A&E in cases of emergency. Telephone bookings for appointments on the day will be available from 8.00am.

 

Chaperone

We will ensure that patients are offered a chaperone for all intimate examinations. Where a chaperone is unavailable, a patient may be asked to return at a clinically appropriate time. We accept that patients may also request a friend or relative to accompany them at these times. Patients may request a staff chaperone from the clinician they see or a member of staff at the surgery.
