New Patients

Thank you for considering registering at  Hazeldene Medical Centre.

The doctors welcome new patients who live within our practice area which covers New Moston, Parts of Moston, Parts of Blackley, Parts of Newton Heath,  Parts of Failsworth and  Parts Chadderton.

As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a registration form available from reception or to download from the website.

  • Please call into surgery to collect forms  or apply via the website.
  • Please allow 48 hours for a reply, please either phone or email us to see if you have been accepted by the practice.
  • If you have you will then be offered an optional new patient medical by our healthcare assistants (covid restriction are currently in place)

Medical treatment is available from the date of registration.  Please contact reception for further information.

Regularly asked Questions: 

Do I need ID and proof of address to register?

NO. You don’t need to provide proof of address or ID, however we may ask to see this to confirm you name when registering if we are struggling to find you digital past / previous GP records.

Does it matter if I don’t know my NHS number?

NO. We might need to ask a few extra questions if we are having difficulty locating your digital past / previous GP records. But we don’t need your NHS number to register you if you don’t know it.

I am an immigrant – can I still register?

YES. provide us with as much details ask you can and we will register you.
