Weight Management Services
Any adult (BMI >28) who wants support with losing weight can be referred to Be Well.
Be Well will work with you to develop a plan based on your personal goals, needs and social circumstances and can connect you to a range of community activities that can help you achieve it. Be Well are the front door to the weight management service.
From June 2021 self-referral has been introduced patients can undertake their own measurement and refer themselves to Be Well by calling 0161 470 7120.
We see the promotion and the maintenance of health as a very important aspect of the service we offer and provide the following services to help our patients. Please telephone or enquire at reception if you want an appointment for any of the clinics.
The partners are all included in the Manchester Health Authority’s Obstetric List and provide maternity services.
The care given to your child in the first five years of life is important for the future. We wish to work in partnership with you to protect your child from disease and promote good health for life. We are also able to treat any health problems that may arise, in the full knowledge of your family’s health and medical history. If you wish to have your child immunised, please make an appointment with either of our practice nurses..
This clinic is run by our health promotion our practice nurses and offers screening for heart disease and advice and support for you about improving your health and preventing disease.
This clinic aims to help people with asthma to understand and manage their condition so that they can maintain the best possible health.
We encourage all our adult female patients to have this simple test at least every three years. You may have a smear taken by your regular doctor, a lady doctor or by the practice nurses.
All women aged 50 to 70 are invited for mammography every three years.
This clinic offers patients with diabetes the opportunity to have regular specialist medical and nursing care.
Advice, information, vaccination and services are provided to help you maintain your health and prevent disease while travelling. There is a charge for most of the vaccinations. Help and advice can also be found at www.fitfortravel.nhs.uk
We undertake a range of minor surgical operations here at the centre that you may be referred to by any of the doctors Dr Paul Taylor runs this clinic with Nurse Karen o. We aim to provide a convenient service without the need to have to travel to hospital.
We offer all our patients aged 75 and over the opportunity to have an annual health assessment at the centre.
Hazeldene Medical Centre is intermittently involved in various research projects ranging from data collection to testing and monitoring new medications for conditions such as diabetes or heart disease. Information on current projects will be posted in the waiting area at the Practice. Please be assured patient confidentiality is maintained at all times and absolutely no information is shared nor any treatments initiated without full patient consent. Any patient considered eligible for any study will be contacted in person and arrangements made for a full discussion. Declining to participate will not in any way affect your medical care.
Walk-in centres offer fast access to health advice and treatment. Mainly nurse-led, they are open to anyone on a drop-in basis. They provide minor treatments, information about local health services and self-help advice.
